Some Law Schools, Medical Schools and Graduate Schools require a Dean’s Certification. The Dean of Students Office will check to see if any disciplinary action has been taken, fill out the form, and mail it to you or to the school that you’re applying to. If you would like to pick up the forms personally, please let the secretary know this when you drop the forms off. The following items/information are needed and should be brought or sent to 3500 WSC: Form (be sure you have signed it—this allows us to release information). If no form is provided, you can provide a note stating that we can release information. Include your name, social security number, and signature. Stamped, addressed envelope. If the form requests a verification of the dates you attended BYU, you can get this information from the Records Office (B-150 ASB). You can either bring it over personally, or the Records Office will mail it with your certification form to us. If you have graduated and need a ranking, contact the Records Office, B-150 ASB (no ranking is available until you have graduated). Please allow at least two (2) working days for these forms to be processed
