Photo Contest Submission Form

Subject to any restrictions noted below, I hereby grant to Brigham Young University and its colleges and departments (collectively “BYU”) permission to use my [TEXT & PHOTOS] creative works submitted in this form, including any writing, text, graphics, illustrations, musical compositions, sound recordings, or audiovisual recording(s), or any other content or data contained therein (collectively “Work”). It is understood, however, that I will retain all copyrights and other intellectual property rights to the Work. In connection herewith, I release BYU and its assignees from any and all claims and demands related to the use of the Work, including any claims for libel or invasion of privacy. BYU and those acting pursuant to its authority shall have the right to use my Work, name and biographical material in connection with this Work, including all rights to reproduce, perform, display and distribute the Work, or edits thereof in whole or in part, in any medium whether now known or yet to be discovered. Moreover, BYU shall not be obliged to make any use of my Work or exercise any of the rights granted herein. I acknowledge that I have read and understand the meaning of this release and that this release agreement is the entire agreement between me and BYU. Furthermore, I represent that I have the full right to enter into this agreement granting the rights herein and that this grant does not violate any rights of any third party.
